THIS AGREEMENT is entered into by and between the PROVAIL Pacific Northwest Adaptive Library, hereinafter called PROVAIL, and any individuals utilizing the Pacific Northwest Adaptive Library. Such individuals will hereinafter be called BORROWER.
WHEREAS:
WHEREAS, BORROWER desires to use certain equipment requested through Pacific Northwest Adaptive Library online check-out process.
WHEREAS, Pacific Northwest Adaptive Library will loan the equipment for borrower’s personal use when available.
NOW, THEREFORE, both parties agree as follows:
CONDITIONS:
1. BORROWER will give equipment left in its custody the same care provided similar property of its own.
2. BORROWER will be held responsible and liable for the equipment loaned to them from Pacific Northwest Adaptive Library. Damage due to abuse or negligence, loss, or theft of a loaned item is the responsibility of the Borrower.
3. BORROWER will be held responsible for all costs associated with shipping to/from PROVAIL, of the equipment during term of agreement and upon return of equipment.
4. BORROWER will be held responsible for full replacement cost if equipment is not returned at termination of agreement.
5. Equipment shall remain in the possession of BORROWER for the time period stated at time of equipment check-out, but may be withdrawn from such use at any time through written notice by PROVAIL. Loan agreement may be extended for an additional period of time if requested by BORROWER and no other request has been submitted by others to obtain said equipment.
6. BORROWER shall return the equipment to PROVAIL on the termination of this Agreement at the following address: 12550 Aurora Avenue North, Seattle, WA 98133.
